Hearing Loss May be a Downside of Viagra

A new study out of the University of Alabama at Birmingham School of Public Health validates the 2007 Food and Drug Administration (FDA) label changes regarding hearing loss and phosphodiesterase type 5 inhibitors (PDE-5i).

PDE-5is medications include Viagra, Cialis, and Levitra. The new study reports the downside of their use is the risk of hearing loss. This downside of Viagra and the other PDE-5is puts the men who use the drugs at twice the risk of hearing loss as men who don’t use the drugs.

The results of Gerald McGwin’s, professor of epidemiology, study are published in the May 18 issue of the journal Archives of Otolaryngology-Head and Neck Surgery

The FDA originally decided to feature warnings about hearing loss on labels for PDE-5i drugs after 29 cases of the problem were reported between 1996 and 2007. The hearing loss was reported to occur within hours to days of taking the drugs.

McGwin’s cross-sectional study used a population-based sample of 11,525 men 40 years or older from the 2003-2006 Medical Expenditure Panel Survey (MEPS), a longitudinal, overlapping panel cohort, with each cohort consisting of approximately 15 000 households, including a subsample of approximately 39 000 individuals chosen from the National Health Interview Survey using a stratified and clustered sample with weights that produce nationally representative estimates.

The overall prevalence of self-reported hearing loss was 17.9% and increased with age. The prevalence among those in their forties was 7.5%, and this increased to 15.0%, 24.4%, 32.9%, and 47.2% in subsequent decades.

Approximately 2% of men reported having used a PDE-5i. Use was lowest among those in their forties (0.8%), steadily increased until men were in their 60s (3.3%), and then subsequently declined to 0.5% for men 80 years or older.

Among those reporting PDE-5i use, Viagra was the most frequently used (80.3%), followed by Levitra (20.2%) and Cialis (12.8%).

The results show that, among 11,525 men over age 40, those who took PDE-5i, were twice as likely to also report hearing loss as were men who had not used the drugs. This relationship was strongest for men reporting use of Viagra.

“”PDE-5i medications work in ED patients by their ability to increase blood flow to certain tissues in the body,” McGwin said. “It has been hypothesized that they may have a similar effect on similar tissues in the ear, where an increase of blood flow could potentially cause damage leading to hearing loss.”

This study represents the first epidemiologic study to evaluate the relationship between PDE-5i drugs and long-term hearing loss. However, it doesn’t find an actually causal relationship, only shows an association. More research is needed to back up the findings.

Dont’s Use Sexual Dysfunction Products With Undeclared Ingredients

The Department of Health strongly urged members of the public not to consume products for managing male impotence from unknown sources following notification from the Hospital Authority of two patients seriously affected by this kind of products found to have contained undeclared ingredients.

A DH spokesman said the affected men, aged 76 and 87 respectively, were found collapsed in late May. Both patients once required intensive care in Mainland China and Hong Kong respectively. They are now under treatment in public hospitals. One of them was still in serious condition and the other in stable condition. Their urine specimen found the presence of undeclared drug ingredients glibenclamide and sildenafil.

Investigation showed that both of them had taken a kind of brick red rhomboid tablets. Laboratory tests on the tablets found at their homes were confirmed to have contained glibenclamide and sildenafil.

Further enquiries revealed that the 76-year-old man had also taken an unnamed blue/white capsule while the 87-year-old had taken an unlabelled blue rhomboid drug. Both of these drugs were found to have contained sildenafil.

In the past two weeks, DH received notification of two other similar cases involving patients taking brown rhomboid tablets. They claimed that the drug that they had taken named “Jiu Bian Wang”(???).

The spokesman said that “Jiu Bian Wang”was not a registered pharmaceutical product in Hong Kong. DH’s earlier laboratory tests on “Jiu Bian Wang”revealed the presence of sildenafil and a high dosage of glibenclamide. DH’s further investigation into these cases is on-going.

Today’s notification brings the number of cases related to unregistered virility products to a total of 58, affecting 56 men aged between 39 and 87. Among them, two patients passed away.

The spokesman said that glibenclamide is a diabetic drug. It could cause nausea and gastro-intestinal upset. Improper use may cause a significant fall in blood sugar level with serious health consequence and even death

The side effects of sildenafil include low blood pressure, headache, vomiting, dizziness, and transient vision disturbances. It may interact with nitrates found in some prescription drugs (such as nitroglycerin) and may lower blood pressure of patients to dangerous levels. Improper use of sildenafil may pose serious health risks, especially for patients with heart problems.

Under the Pharmacy and Poisons Ordinance of Hong Kong, products containing sildenafil or glibenclamide must be registered before sale and can only be sold on a doctor’s prescription and under supervision of a pharmacist.

The spokesman urged the public not to consume virility products of unknown or unclear sources, because the efficacy, safety and quality of such could not be assured, the spokesman said.

“Members of the public and the trade who have in their possession similar products are urged to stop consuming or trading them immediately,” he said.

They should destroy and dispose of the products or submit them to the Department’s Pharmaceutical Service at 3/F, Public Health Laboratory Centre, 382 Nam Cheong Street, Kowloon during office hours.

“People who have problems of sexual dysfunction should consult medical professionals for appropriate advice or medication,” the spokesman said.

Hispanic Men Face Elevated Risk Of Erectile Dysfunction

Hispanic men between the ages of 20 and 50 are two and one half times as likely to develop erectile dysfunction (ED) as men of other races and ethnicities, according to a 2005 study of 2126 men published in the American Medical Association’s “Archives of Internal Medicine”.

Put another way, study results show 12.5% of Hispanic men in this age range are afflicted with this condition, and when the population is expanded to include older Hispanic men, the rate rises to 40%, or twice that of other ethnic groups. These study results control for lifestyle and health conditions, such as diabetes, that contribute to ED, and evidence the same outcome when Mexican American and other Hispanic groups are considered in isolation. Doctors find no conclusive explanation for the higher risk Hispanics face of developing ED, citing a need for further research.

Of late there has been no lack of interest in treatments for erectile problems. Dangerous Problems with Counterfeit Viagra

The world, including the US, is being flooded with counterfeit Viagra and other erectile dysfunction medications. A report in the Feb 12, 2009 issue of the New England Journal of Medicine highlights the dangerous problems.

Counterfeit Viagra DangersThe article reports on a total of 150 non-diabetic patients with severe hypoglycemia (low blood glucose) were admitted to the five public hospitals in Singapore. The admissions were between January 1 and May 26, 2008. All the patients were men except one, ages ranged from 19 to 97 years (median, 51). Seven patients remained comatose as a result of prolonged decrease glucose to the brain,and four subsequently died.

Several patients (45/150 or 30%) admitted to ingesting illegal sexual-enhancement drugs before the onset of hypoglycemia. Samples of the drugs obtained from these patients and from drugs seized in police raids were analyzed by means of high-performance liquid chromatography. It was found that many of the preparations were contaminated with Glyburide (a powerful medication used to treat diabetes).

So people thought they were taking Cialis or Viagra. What they were often taking were herbal preparations (e.g. Santi Bovine Penis ErectingCapsule) that contained small amounts of the active ingredient (Cialis or Viagra), but which also contained glyburide.

Since none of the men were diabetic, they experienced a severe drop in blood sugar (hypoglycemia). Seven men sustained serious brain damage due to low blood sugar, of which four ultimately died.

Harmful ingredients found in counterfeit medicines include boric acid, leaded road paint, floor polish, shoe polish to get the sheen on the tablet, talcum powder, cement powder, chalk and brick dust, nickel and arsenic. Any active drug ingredient is usually minimal or simply wrong – one supply batch of fake Viagra was in fact amphetamine and not Viagra (sildenafil).

Hong Kong Urges Not To Consume Sexual Dysfunction Managing Products

Hong Kong Department Of Health appealed to members of the public not to buy or use a product named “Chong Cao Ju Wang” as it was found to contain an undeclared drug which is known to link with serious side effects.

A DH spokesman said a 69-year-old man was admitted to United Christian Hospital on July 23 after he was found unconscious at home. His blood glucose level was very low on admission.

Investigation revealed that he had a history of taking the above mentioned product purchased at a drug shop in Shenzhen. The urine of the patient was found to have contained glibenclamide (a diabetic drug) and sildenafil (a drug used for treating male sexual dysfunction).

Today, laboratory tests on one remaining capsule of the product provided by the patient showed the presence of sildenafil.

The spokesman said glibenclamide could cause nausea and gastro-intestinal upset. Improper use may cause a significant fall in blood sugar level with serious health consequence and even death.

Back pain treatment could solve erection problems

Men who experience back pain and lower spine problems could also experience problems with erections. Irwin Goldstein, M.D., medical director of Alvarado Hospital’s Sexual Medicine Program—the first hospital-based sexual medicine program in the U.S.—and editor-in-chief of The Journal of Sexual Medicine, recently discovered that treating one patient’s back pain also cured his erection problems.

The patient had experienced a car accident, and complained of leg numbness, back pain and erectile dysfunction. “After completing the initial workup, we determined the problem was the patient’s pelvic floor muscles,” Dr. Goldstein said. “We referred him to one of the hospital’s physical therapist who had advanced training in pelvic floor rehabilitation.” Treating the back pain resulted in improvement in erectile dysfunction.

Erectile dysfunction drugs may not always be needed. Back pain treatment could improve erectile dysfunction long-term. Back injuries can cause trapping of blood. Men with back pain may be experiencing blood flow difficulties in the pelvis as the cause of erection problems. According to Dr. Goldstein, pelvic muscles can become weak and spasm from back injuries, resulting in decreased sexual function from limited blood flow to the penis when muscles spasm.

Muscle energy, core and lumbar strengthening exercises to align the pelvis and relax the pelvic floor muscles, performed by Physical therapist and pelvic floor specialist Kerri Krebs, facilitated recovery. Following treatment, erectile function and leg numbness improved for the patient, who had seen several other physicians and also had been prescribed erectile dysfunction medication.

Orthopedic spine specialist Ramin Raiszadeh, M.D warns however that men with back or pelvic pain, or who experience urinary incontinence need to see a specialist as soon as possible. Erection problems can be caused by …”other medical conditions, including cardiovascular disease, diabetes, infection, inflammation and tumors can cause these symptoms as well.”
